Informed Consent Form

I understand and agree that If I experience any pain or discomfort during the session, I will immediately inform Colleen. I also understand that Colleen reserves the right to refuse my participation in the sessions, for any reason she deems necessary. I hereby give my consent to participate in this virtual energy healing session and understand that the services provided by AlchemizMe and Colleen Master are intended to reduce stress and support my own healing. I understand and agree that the services provided during energy healing session do not constitute nor substitute traditional medical treatment or advice. I understand that AlchemizMe and Colleen cannot diagnose, treat or recommend treatment for any medical or mental health conditions. I acknowledge that I must continue to my healthcare and I should contact a certified and licensed medical or mental health care professional for any physical or psychological conditions or concerns that I may have. I agree and understand that my participation in this Energy Healing Session is voluntary and that at any time during the Session I can choose to end my participation. I also understand that I may experience 'self-healing reactions' during the 48 hours following the session. I understand and agree that by providing this informed consent I am assuming full responsibility for participating in this Energy Healing Session and I hold harmless both Colleen Master and Alchemize Me LLC dba AlchemizMe. Acknowledgement of Cancelation and Refund Policies

I acknowledge and agree that once I register, there is a no refund policy. I further acknowledge and agree that Colleen can revoke my participation at anytime for any reason she deems necessary and a refund will not be issued. In the event of a crisis or emergency situation, I will inform Colleen of my absence or lack of participation. And further acknowledge and agree that Colleen will consider each emergency and crisis on a case by case basis for makeup sessions or reimbursements.
